Comments:
This inspection was conducted by licensing staff using and alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated on September 22, 2021 and concluded on September 29, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 2 children present directly supervised by 1 staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. The children were observed resting during the inspection. The inspector reviewed on one child?s record, one staff record, and three household records.


Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on children records review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #1 (date of enrollment 3/10/2021) did not have a physical examination in record available for review during the inspection.

Plan of Correction: The child's parent get doctor's appointment next week; next week I fixed.

Standard #: 22VAC40-111-60-B
Description: Based on children record review, the family day home operator did not contain emergency contact information including addresses.
Evidence: Child #1 (date of enrollment 3/10/2021) did not have two designated person contact addresses to contact in case of emergency fi the parent cannot be reached. The provider had only one address listed in the child's record for emergency contact.

Plan of Correction: I will fixed today.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on staff and household records review, the family day home operator and person living in the family day home did not have most recent central registry findings.
Evidence:
1. Staff #1 central registry findings expired on 5/02/2021.
2. Household member #1 central registry findings expired on 5/02/2021.
3. Household member #2 central registry findings expired on 5/02/2021.
4. Household member #3 central registry findings expired on 5/02/2021.

Plan of Correction: I sent paper Virginia state police
I didn't get the results yet.
