A blank copy of the VDOE model form: AUTHORIZATION TO APPLY A NON-PRESCRIPTION TOPICAL SKIN PRODUCT was sent to the Provider, per request.

Comments:
A monitoring inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.

This inspection was initiated and concluded on 11/04/2021. The Provider was contacted and a virtual inspection was conducted. There were 6 children present (18 points), ranging from 7 months to 3 years in age, with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 8 children records, 5 household records, and 2 staff records were reviewed.

Information gathered during the inspection determined that there was (1) non-compliance with an applicable standard. This violation was documented on the Violation Notice issued to the program.

Standard #: 22VAC40-111-830-B
Description: Based on review and interview, a shelter-in-place drill was not practiced a minimum of twice a year.

Evidence -
1. Based on the information recorded on the Family Day Home emergency response drill log, a shelter-in-place drill has not been recorded during the current year of 2021.
2. The Provider stated (during an interview conducted on the date of inspection 11/04/2021) that while a fire drill has been conducted for each month, a shelter-in-place drill has yet to be conducted in 2021.

Plan of Correction: A shelter-in-place shall be conducted with the children based on the procedures laid out in the Family Day Home's Emergency Preparedness and Response Plan.
