An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted today. Observed 1 child(ren) (3 points) directly supervised by 1 caregiver. Ratio and supervision were in compliance. Operational responsibilities, children's information, caregiver training and development, parental involvement and notifications, nutrition and food services, emergencies and emergency drills, and transportation and field trips were reviewed. The child was engaged in various activities, to include: dancing, riding on the toy horse, watching TV and demonstrating his love for washing his hands with soap and water. An area of non-compliance is identified in this report. The exit interview was conducted with the Provider. Inspection conducted 9:20am to 11:15 am. Standard #: VENDHOM-000-010-B Description: Based upon interview with provider, required documentation was not available on record. Evidence: Child A( start date 5/23/2017) did not have an immunization record on site. Plan of Correction: Provider has already asked mom to return required enrollment and immunization paperwork. Today is the child's first day.
Standard #: VENDHOM-000-012-B Description: Based upon record review and interview, required training was not on record. Evidence: Provider has not completed the 10 hour preservice training as required vendor agreement. Plan of Correction: Provider will complete training as required. Ramadan is beginning May 27th-July 31st.and will complete after the holiday has passed.
Standard #: VENDHOM-000-013-C-2 Description: Based upon observation, outlets were missing protective covers. Evidence: Outlet covers in the dining room/ living room and tv room were missing out covers. Plan of Correction: Provider will replace outlet covers during the inspection.
Standard #: VENDHOM-000-026-E Description: Based upon observation, non absorbent diaper changing surface was not available at the time of inspection. Plan of Correction: Provider will purchase a diaper changing pad for the changing table.
Standard #: VENDHOM-000-029-B Description: Based upon record review and interview with provider, required addition of the "Lock down" procedures were not added to the emergency preparedness plan. Plan of Correction: Provider will watch the training videos and add required Lock down drill scenario to plan and practice 1 time annually and document on a drill log.
