Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A renewal inspection was initiated and concluded on 12/07/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 9 children present, ranging in ages from 18 months to 3 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records, 2 household member records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:

Standard #:	8VAC20-800-700-A
Description:	Based on observation and interviews it was determined that not all requirements for medication administration were followed. Evidence: 1. An emergency medication was onsite for child A without a written authorization from the parent. 2. The family day home's written medication policy states that prescription or nonprescription medications will not be administered in the family day home.
Plan of Correction:	The provider will request the parent of child A complete a medication authorization form for the emergency medication. Once the provider has obtained Medication Administration Training (MAT) the medication policy will be updated and signed by the parents of children in care.
Standard #:	8VAC20-800-730-D
Description:	Based on observation it was determined that not all medication was kept in a locked place. Evidence: An emergency medication for Child A was observed unlocked in a book bag.
Plan of Correction:	The medication was locked during the inspection.
Standard #:	8VAC20-800-180-A
Description:	Based on review of two household members it was determined that not all records contained documentation of a negative TB (tuberculosis) screening every two years. Evidence: Records contained documentation of positive TB screenings that required follow up with a physician or chest x-ray for household member A (dated: 01/27/2021) and household member B (dated: 01/28/2021). There was no documentation that a follow up was performed to document the absence of TB for household member A or household member B.
Plan of Correction:	The provider has requested that household member A and household member B obtain documentation of the absence of TB and will submit the results to the provider.
Standard #:	8VAC20-800-220-A
Description:	Based on interviews it was determined that not all staff obtained the required medication administration training (MAT) when the provider has agreed to administer medication. Evidence: The provider accepted an emergency medication from the parents of Child A. The provider is not currently MAT certified.
Plan of Correction:	The provider has scheduled a medication administration training.
