Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and a virtual tour of the program (if applicable).

A monitoring inspection was initiated and concluded on 2/16/2022. The provider was contacted by telephone and a virtual inspection was conducted. There were 11 children present (32 points), ranging in ages from 8 months to 3 years, with the provider and two assistant caregivers supervising. The Inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. A total of 2 children's records and 2 caregiver records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and one violation was documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-180-A
Description: Based on a review of caregiver health records, it was determined that one TB Health Screening was not repeated every 2 years.
Evidence:
The provider's TB Health Screening expired on 10/17/2021.

Plan of Correction: I will get a new health screening done.
