Comments:
An unannounced monitoring inspection was conducted today between 11:05am and 11:40am. There were 7 children present with the Provider and 1 assistant for a total of 24 points within the supervision guidelines. The children were observed eating lunch and dancing to music. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Standard #: 8VAC20-800-800-B
Description: Based on record review, the providers emergency preparedness plan was not reviewed at least annually.
Evidence: The most recent documented review of the emergency preparedness plan is dated 1/15/2021.

Plan of Correction: Corrected during inspection.

Standard #: 8VAC20-800-480-D
Description: Based on observation and interview, equipment used by children were not maintained and used in accordance with the manufacturer's instructions.
Evidence: During the inspection a child was observed sitting in a feeding chair and the seat buckle was not buckled.

Plan of Correction: The child was buckled but he pulled it off.
