Technical Assistance:
Technical assistance was offered related to: Background checks, hand sanitizers, crib sheets; staff and children's records.

Comments:
An in person renewal inspection was initiated on 2/16/22 and concluded on 2/24/22. There were 7 children (18 points) in direct care and supervision of 2 caregivers. The children were observed engaged in circle time activities. A walk through of the family day home was conducted to assess the indoor and outdoor space, and cribs used by the children. There were no medications on site. 3 children's records and 2 caregiver's records were reviewed, in addition to other daycare documents and emergency supplies.

Violations:
Standard #: 22.1-289.035-B-4
Description: Based on records review and staff interview, the provider did not obtain results of an out of state central registry and sex offender search for an employee who has resided outside of Virginia within the last 5 years.
Evidence:
1. Caregiver # 1's VA central registry form indicated that she resided in Maryland.
2. Caregiver # 1 stated that she lived in Maryland for approximately three months at the end of 2019 and the beginning of 2020 before she moved to Virginia.

Plan of Correction: The Maryland central registry will be submitted and the sex offender search will be completed. The licensing inspector will be notified as soon as the results are obtained.

Standard #: 8VAC20-800-800-B
Description: Based on record review, the provider did not review the emergency plan annually.
Evidence: The most recent date that the emergency plan was reviewed was on 2/17/20.

Plan of Correction: The plan will be updated annually and documented.

Standard #: 8VAC20-800-100-A
Description: Based on record review, the provider did not obtain documentation of a physical exam for children in care within 30 days of enrollment.
Evidence:
1. Child # 2's first day of enrollment was 3/17/17.
2. Documentation of a physical exam for Child # 2 was not on file.

Plan of Correction: The physical will be obtained.

Standard #: 8VAC20-800-90-A
Description: Based on records review, the provider did not obtain immunization records for children in care before their first day of enrollment.
Evidence:
1. Child # 1's first day of enrollment was 8/9/21.
2. Child # 1's record did not have the immunization record on file.

Plan of Correction: The immunizations will be obtained.

Standard #: 8VAC20-800-500-F
Description: Based on observation and staff interview, the family day home had a trampoline during the hours that children were in care.
Evidence: A medium sized trampoline was found in the backyard. The provider stated that she was not aware that trampolines could not be used for the children.

Plan of Correction: The trampoline will be removed.

Standard #: 8VAC20-800-530-B
Description: Based on observation, cribs used by children did not have tight fitting bottom covers.
Evidence: Two cribs were observed to have crib sheets that were too large for the crib mattress. The extra material was creating folds in the crib sheets.

Plan of Correction: Proper fitted sheets will be used.
