This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts and documents review.
A renewal inspection was initiated on 2/15/2022 and concluded on 2/26/2022. The provider was contacted by email and a virtual inspection of documentation was completed. The provider has been closed since March 2020 are currently no children enrolled in the family day home. One staff record was reviewed for completeness.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Standard #: 22.1-289.036-A Description: Based on record review and interview, the provider has not obtained a repeat central registry background check every five years.
Evidence: The providers most recent central registry background check is dated 6/25/2015. This was cited at the providers last inspection on 3/5/2021.
Plan of Correction: I have put a new background check in the mail.
