Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the code compliance inspection. Subsidy standards that are not covered in code were reviewed during this SHSI. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/29/2025. A POC submitted after this date will not appear on the public website.

Violations:

Standard #:	22.1-289.035-B-3
Description:	The vendor is required to request a child abuse and neglect central registry search prior to hire for all staff. Staff #2 has been employed for more than three weeks and has not yet had a central registry search request submitted.
Plan of Correction:	Staff #2 has been sent home and will not be allowed to return to work until they provide the report for the central registry search request.
Standard #:	8VAC20-790-560-A
Description:	The vendor is required to obtain documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children for all staff. Staff #2 has been employed for over three weeks and has not yet submitted documentation of a negative TB screening.
Plan of Correction:	Staff #2 has submitted a negative TB screening report. Please see the attached copy.
Standard #:	8VAC20-790-630-C-2
Description:	Protective covers are required on all electrical outlets in areas used by children of preschool age or younger. There were four outlet covers missing in the two to three year old classroom and nine outlet covers missing in the three to four year old classroom.
Plan of Correction:	750 pieces of outlet cover was purchased from Amazon and has been delivered to the center. A copy of the receipt is attached. All the outlet missing covers at the time of inspection has been covered.

Description:	The vendor is required to have one working, battery-operated flashlight. There was no flashlight on site.
Plan of Correction:	A flashlight was purchased from Amazon and has been delivered to the center. A copy of the receipt is attached.

Description:	The vendor is required to have one working, battery-operated radio. There was no battery-operated radio on site.
