Technical Assistance:
Discussion on the status for the VA CPS check for Household Member #2. This request of the central registry was sent in October, and has been followed up by the Provider multiple times and documented as such. When this finding is obtained it will be shared with the Licensing Inspector.

Provider confirmed that a carbon monoxide detector is install on site within the Family Day Home.

Comments:
A monitoring inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.

This inspection was initiated and concluded on 03/15/2022. The Provider was contacted and a virtual inspection was conducted. There were 4 children present (12 points), ranging from infant to preschool-age, with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 6 children records, 1 staff record, and 2 household records were reviewed.

Information gathered during the inspection determined that there was (1) one non-compliance with an applicable standard. This violation was documented on the Violation Notice issued to the program.

Standard #: 8VAC20-800-100-A
Description: Based on review, 2 of 6 children files indicated that the Provider did not obtain documentation of a physical examination (by or under the direction of a physician) within 30 days after the first day of attendance.

Evidence -
1. On the date of inspection (03/15/2022), the following were without documentation of a physical examination on file at the Family Day Home: Child #2, and Child #4.
2. The start date for Child #2 was listed as 01/03/2022. The start date for Child #4 was listed as 09/19/2021.

Plan of Correction: The parents will be contacted to obtain these physicals.
