Comments:
A subsidy health and safety (SHSI) inspection was conducted on 03/22/2022. There were 3 children present with the provider. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-790-180-B
Description: Based on documentation review and interview with the provider, an updated written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs was not mantained.
Evidence:
1) On the date of inspection, the inspector observed 3 children in care, however, the attendance record documented 5 children in attendance. 2) The provider stated that 2 of the children were in attendance early in the morning but were picked up by the school bus and would be back in the afternoon, after the school bus drops them up. The inspector advised the provider to note on the attendance sheet each time the children leave for and come back from school.

Plan of Correction: Will document each time the children leave the family day home.
