An unannounced joint complaint investigation was conducted with the local agency and provider to investigate allegations regarding supervision, documentation of injury, notification to parents, behavioral guidance, care, protection, and guidance. Interviews were conducted and documentation was reviewed. The allegations were determined to be invalid due to insufficient corroborating evidence.

Evidence: Child A's record did not have two emergency contacts. Plan of Correction: The provider will ensure that all children's records have two emergency contact.
