Comments:
An unannounced renewal inspection was conducted today between the hours of 9:30 am through 11:00 am. There were a total of eight children (16 points) observed with -two caregivers. A total of four children's files and -two staff/household member's files were reviewed. The children were engaged in the following: toy play, games, circle time, outside time, and other organized activities. Areas of non-compliance are identified in the violation notice. Standard #: 22.1-289.058
Description: Based on observation and interview, the family day home was not equipped with at least one carbon monoxide detector.
Evidence:
1. There was not a carbon monoxide detector in the family day home.
2. Staff #1 confirmed that the family day home did not contain a carbon monoxide detector.

Plan of Correction: The provider will buy one.

Standard #: 8VAC20-800-760-A-1
Description: Based on observation, the first aid kit did not contain required supplies.
Evidence:
1. The first aid kit was missing adhesive tape, antiseptic cleaner, and tweezers.

Plan of Correction: The provider will buy the missing supplies and put them into the first aid kit.

Description: Based on observation, the family day home did not contain a working battery operated weather band radio.
Evidence:
1. The family day home did not have a working battery operated weather band radio. The provider did not have batteries for the weather band radio.

Plan of Correction: The provider will purchase batteries for the weather band radio.

Standard #: 8VAC20-800-800-A
Description: Based on documentation review and interview, the family day home did not have a written emergency preparedness and response plan.
Evidence:
1. The family day home did not have a written emergency preparedness and response plan.
2. Staff #1 confirmed that the family day home did not have a written emergency preparedness and response plan.

Plan of Correction: The provider will complete the department's model form for the Emergency Preparedness and Response Plan and post it on the family board.

Description: Based on documentation review, children's records did not contain required information.
Evidence:
1. Child # 2's (Start date-11/29/21) record was missing the parent's signature for written authorization for emergency care.
2. Child #4's record (Start date-07/27/18) was missing documentation of the review of the child's emergency contacts.

Plan of Correction: The provider will have the parents to sign the children's records. The provider will ensure that each child record is complete by conducting a monthly review of each record.
