Comments:
A monitoring inspection was conducted on 4/27/2022. 10 children (22pts) were in attendance with 2 caregivers. The physical space, 2 children's records, Caregiver records, posted documents, evacuation drills, attendance records and other required documents were reviewed. No medications were on site. Children were observed interacting with caregivers on the playground. Age appropriate projects were hanging on the walls and developmentally appropriate materials were available for the children. No medications were on site. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description: Based on record review and interview with the Provider, a follow up Tuberculosis screening was not conducted within two years from the last screening. Evidence: The Tuberculosis screening for Caregiver A expired 11/18/2021.

Plan of Correction: The TB screening will be done right away.
