Discussion was held on the topics of current enrollment, current staffing, and training hours.

Comments:

This monitoring inspection was initiated by licensing staff and concluded on 04/21/2022, between the hours of 9:30 am to 12:30 pm. There were 12 children present (34 points), ranging from infant to preschool-age, with 3 caregivers supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 12 children records, and 3 staff records were reviewed. Information gathered during the inspection determined that there were two (2) non-compliances with an applicable standard, and code. Standard #:	22.1-289.035-A
Description:	Based on review and interview, 1 of 3 staff files indicated that it has been more than five years since the last search of the central registry (CPS check) from the Commonwealth of Virginia. Evidence - 1. On the date of inspection (04/21/2022), the current CPS check on file for Staff #2 was dated - 03/31/2017. 2. The Provider stated (during an interview conducted on the date of inspection) that she was aware of this documentation and was currently in the process of getting it updated.
Plan of Correction:	This form has been obtained and completed. After it is notarized, it will be sent in to OBI.
Standard #:	8VAC20-800-90-C
Description:	Based on review, 1 of 12 children files indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations (for a child who was not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years. Evidence - 1. On the date of inspection (04/21/2022), the most current Immunization page on file for Child #4 was dated - 06/15/2021. 2. Based on information contain within the Child's Record, on the date of inspection, Child #4 was under 2 years of age.
Plan of Correction:	The parent has been contacted and an updated immunization page will be obtained.
