Technical Assistance:
Discussed the use of the outside play equipment. the climber platform is over 4 feet high so the children will not use the climber, only the swing. The outside play equipment will be removed at a later date and replaced with the appropriate size.
The indoor slide platform is 3 feet high and currently sits on top of 2 pieces of carpet. This LI suggested that the provider add a layer of rubber mats or tiles between the two carpets to provide sufficient surfacing.

Comments:
A monitoring inspection was initiated on 5/6/2022 and concluded on 5/6/2022 from 10:15 am until 11:30 am. There were a total of 10 children ( 27 points) observed with 2 caregivers present. The children were engaged in the following: toy play, computer touch screen games, puzzles, shapes, diapering and rest time. A total of 3 child records and 3 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-800-750-A
Description: Based on observation and provider interview, when topical skin products such as diaper ointment and lotion, are used, written parent authorization shall be obtained at least annually.
Evidence: There were multiple diaper creams observed without parent authorizations. The provider stated that she has not obtained written authorization to apply the diaper creams.

Plan of Correction: The provider will have the parents complete the model form for diaper cream authorization.
