Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and training. The Provider stated that she recently obtained her certificate of completion for MAT. However, this certificate could not be found at the time of inspection. Please forward this document to the Licensing Inspector for his notes.

Discussion was also held on the future start of a new assistant. As a reminder, a Fingerprint Check must completed and the findings be on site at the FDH prior to this new employee's start date. All other required documentation will also need to be on site for this new staff person according to the applicable timeline.

Comments:
This monitoring inspection was initiated by licensing staff and concluded on 05/20/2022, between the hours of 1 pm to 2 pm. There were 11 children present (26 points), ranging from infant to preschool-age, with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 12 children records, 1 household record, and 2 staff records were reviewed.

Information gathered during the inspection determined that there was 1 (1) non-compliance with an applicable standard. This violation was documented on the Violation Notice issued to the home.

Standard #: 8VAC20-800-90-C
Description: Based on review, 1 of 12 children files indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for a child (who was not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years.

Evidence -
1. On the date of inspection (05/20/2022), Child #7 had an immunization page dated - 10/15/2021.
2. On the date of inspection, based on the information on file, Child #7 was under 2 years of age.

Plan of Correction: An updated immunization page will be obtained from the parents of Child #7 and placed on file.
