Technical Assistance:
Technical assistance was provided in the following areas:
* The covering material of a column in the care area and one of the tables are starting to show signs of wear and tear. The provider should consider fixing or replacing them.
* The manufactured date of the crib in the infant area could not be verified. The provider must find the documentation and provide to the inspector.
* Provider will review art supplies to ensure they are not toxic. Art materials and other potentially hazardous materials will be stored away from children's reach.
* Maintenance on the area rugs to prevent them to be tripping hazards.

Comments:
An announced Initial inspection was conducted on 07/07/2022 between the hours of approximately 9:00 am and 12:30 pm. There were no children in care on this date.

The Inspector reviewed 4 staff records, 2 household member records and 4 children's records. During this initial inspection, the entire physical space of the family day home was reviewed along with emergency supplies, equipment, the family day home's forms with policies and procedures, and the Emergency Preparedness Plan were reviewed. The care area looks clean and well organized with ample space and variety of toys and learning materials for the children. The inspector also walked with the provider to the community playground which the provider plans to take the children to for outdoor play. The permanent climbing structure on the playground is not age appropriate for the children in care, so the inspector advised the provider that the children shall not use this structure.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-800-100-A
Description: Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1) The records for Child #2 (start date 10/05/2020) and Child #3 (start date 08/31/2020) did not contain documentation of a physical examination.

Plan of Correction: Fixed it.

Standard #: 8VAC20-800-120-B
Description: Based on record review, staff's records did not contain all required information.
Evidence:
1) The record for Staff #4 did not contain documentation that two or more references as to character and reputation as well as competency were checked before employment.

Plan of Correction: I provided staff with a reference sheet to be filled out.

Standard #: 8VAC20-800-60-B
Description: Based on record review, children's records did not contain all required information.
Evidence:
1) The record for Child #1 did not contain the telephone number of one custodial parent's place of employment and the address of a second custodial parent's place of employment.
2)The record for Child #4 did not contain the name, address and telephone number of one custodial parent's place of employment, and the address of one designated person to contact in case of an emergency if the parent cannot be reached.

Plan of Correction: Fixed it.

Standard #: 8VAC20-800-90-A
Description: Based on record review, the provider did not obtain documentation that the child has been adequately immunized before the child may attend the family day home.
Evidence:
1) The records for Child #2, Child #3 and Child #4 did not contain documentation of immunizations.

Plan of Correction: Fixed it.

Standard #: 8VAC20-800-200-A
Description: Based on record review, the provider did not ensure to orient the substitute provider and assistant by the end of their first week of assuming job responsibilities.
Evidence:
1) The records for Staff #2, Staff #3 and Staff #4 did not contain documentation of having completed orientation training.

Plan of Correction: Fixed it.

Standard #: 8VAC20-800-690-D
Description: Based on observation, the diapering surface did not meet all requirements.
Evidence:
1) The diapering mat had small rips which prevented it from being nonabsorbent and washable.

Plan of Correction: Fixed it.

Standard #: 8VAC20-800-690-E
Description: Based on observation, a foot-operated disposal system was not available to dispose of soiled diapers.
Evidence:
1) A foot-operated trash can was not available to dispose of soiled diapers in the diapering area.

Plan of Correction: Fixed it.
