Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and subsidy training.

Comments:
This Subsidy Health and Safety Inspection (SHSI) was initiated by licensing staff and concluded on 07/07/2022, between the hours of 9 am to 12 pm. There were 9 children present (24 points), with 3 caregivers supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined that there were 2 non-compliance with applicable standards. Violations were documented on the Violation Notice issued to the home.

Standard #: 8VAC20-790-250-B
Description: Based on review and interview, 1 of 3 staff files indicated that within the first 90 days of employment or service a caregiver did not complete Virginia Preservice Training for Child Care Staff sponsored by the Department of Education.

Evidence -
1. On the date of inspection (07/07/2022), a certificate of completion was not available for Staff #2 in reference to the Virginia Preservice Training for Child Care Staff (10 hour course).
2. The start date for Staff #2 was listed as 03/30/2022..
3. The Provider stated (during an interview conducted on the date of inspection) that Staff #2 is currently in the process of completing this course.

Plan of Correction: All training will be completed and certificate will be placed on file.

Standard #: 8VAC20-790-250-F
Description: Based on review, 2 of 3 staff files indicated that a certificate of completion was not available for the (3 hours) department's annual health and safety update course.

Evidence -
1. On the date of inspection (07/07/2022), a certificate of completion for the annual health and safety course was not available for review for: Staff #1 and Staff #3.
2. Both Staff #1 and Staff #3 have been employed by the FDH for more than 1 year.

Plan of Correction: All training will be completed and certificate will be placed on file.
