As discussed, please ensure that all background checks are obtained for all staff.

As discussed please submit your renewal application prior to the expiration date of your license.

Comments:
An unannounced Renewal Inspection was conducted today. A total of 10 children (27pts) were in attendance with the Provider and 1 Assistant. The children were washing hands and preparing for lunch time at the beginning of the inspection. Staff were observed feeding the younger children and the Provider was assisting with having the older children sit at tables and portable high chairs where they ate their lunch. Lunch today was spaghetti and slices of watermelon with milk to drink. After lunch, the children transitioned to nap time. Staff were observed assisting children to their cots and placing infants in their designated cribs. No food allergies identified and no medication needs at this time. A total of 4 children's records, all staff/household records were reviewed. Previous violations found at the last inspection were reviewed and corrections were verified. Description: Based on staff record review, a fingerprint background search was not completed.
Evidence:
A fingerprint background search was not completed for Staff #2 prior to hire.

Plan of Correction: This staff will schedule a fingerprint appointment as soon as possible. The search results will be forwarded to the Licensing Inspector within 10 working days for review.

Standard #: 8VAC20-770-60-C-2
Description: Based on staff record review, a Central Registry search request was not obtained for one staff.
Evidence:
A Central Registry search was not requested for Staff #2.

Plan of Correction: I will send a Central Registry search for this staff as soon as possible. The search results will be forwarded to the Licensing Inspector within 10 working days for review.

Standard #: 8VAC20-800-120-A
Description: Based on review of staff record, one record was not maintained.
Evidence:
A staff record was not maintained for Staff #2.

Plan of Correction: I will maintain a record for this staff with all the required documentation.

Standard #: 8VAC20-800-90-A
Description: Based on child record review, documentation that two children have been adequately immunized was not obtained before prior to care.
Evidence:
Child #1 and #2 began care on 04/11/2022. Documentation that both children were adequately immunized was not obtained until 05/09/2022.

Plan of Correction: If a child's doctor does not complete the Virginia health form, I will require a statement that a child has been adequately immunized. I will obtain this before any child can begin care.
