Comments:
An in-person renewal inspection was conducted on July 20, 2022. There were 5 children present with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Children were observed playing outside. A total of 2 child records, 1 household member record and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-770
Description: Based on observation, there was not a working portable battery-operated weather band radio.

Evidence: There were not batteries for the portable battery operated weather band radio.

Plan of Correction: We will make sure to have batteries.

Standard #: 8VAC20-800-80-A
Description: Based on record review, the provider did not obtain proof of identity from the parent within seven days of the child's first day of attendance at the family day home.

Evidence: Child 1 and 2 did not have a completed proof of identity.

Plan of Correction: I will make sure to sign that I viewed the originals

Standard #: 8VAC20-800-210-A
Description: Based on record review, caregivers did not obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Evidence: Caregiver 1 obtained 14 hours of training. Caregiver 2 did not obtain training hours in 7/2021-7/2022.

Plan of Correction: We will make sure to complete 16 hours of training next year.

Standard #: 8VAC20-800-530-I
Description: Based on observation, a mattress was not covered with a waterproof material that can be cleaned and sanitized.

Evidence: The plastic covering of a crib mattress was observed to be ripped and therefore not able to be cleaned and sanitized sufficiently.

Plan of Correction: We will get a new mattress.
