Comments:
An unannounced Monitoring inspection was conducted onsite today. A total of 11 children (24points) with the Provider and 1 assistant were present. All but one child was observed napping in both the portable bed room and crib room. One child(age 5) was observed interacting quietly with an activity. No medication needs at this time. No food allergies identified. The home was equipped with toys and equipment determined age appropriate for the children currently in care. Observation of the backyard was made that included the play equipment. A total of 4 children's records, staff and household records were reviewed. Previous violations from the last inspection were reviewed and corrections were verified. This inspection occurred at approximately 11am and ended at 1pm. Thank you for your cooperation today. Description: Based on child record review, annual review of emergency contacts were not completed.
Evidence:
Child #1, #2 and #3 emergency contacts were not reviewed during the past 12 months.

Plan of Correction: All children's records will have their emergency contacts reviewed on the same day to ensure an annual review is completed.

Standard #: 8VAC20-800-80-B
Description: Based on child record review, the provider did not document verification of one child's identity within 7 days of enrollment.
Evidence:
Child #4 began care on 01/04/2022. The provider did not document who viewed the proof of identity and the date in which it was reviewed.

Plan of Correction: I will ensure that the date and my name is documented on all proof of identities ongoing.

Standard #: 8VAC20-800-170-A
Description: Based on review of household records, an initial TB risk assessment was not obtained.
Evidence:
Household member #1 has not received a TB risk assessment.

Plan of Correction: This household member will have a TB screening completed as soon as possible.

Standard #: 8VAC20-800-180-A
Description: Based on staff record review, one staff did not obtain a subsequent TB screening every 2 years.
Evidence:
Staff #2's last TB screening was dated June 2020.

Plan of Correction: This staff will have a new TB screening completed as soon as possible.
