Comments:
An unannounced renewal inspection was conducted today between the hours of 10:00 am through 11:15 am. There were a total of five children (13 points) observed with two caregivers. A total of four children's files and four staff/household member's files were reviewed. The children were engaged in the following: toy play, games, puzzles hand washing, and other organized activities. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 8VAC20-800-700-B
Description: Based on record review, a child's prescription medication did not have written authorization from the parent or physician.
Evidence:
1. Child #1 (Start date-01/30/21) has epinephrine on site and has a diagnosed food allergy. There was no written authorization from the parent or physician.

Plan of Correction: The child is going to the doctor today. The provider will give the parents the necessary documentation and have the parent and physician complete the documentation.

Standard #: 8VAC20-800-90-C
Description: Based on record review, the family day home did not obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements.
Evidence:
Child # 2's record (Start date-11/01/21) did not contain documentation of additional immunizations. The last documentation of an immunization was dated 10/01/21.

Plan of Correction: The provider will ask the parents for a copy of the most recent immunization records and put it in the record.

Standard #: 8VAC20-800-210-A
Description: Based on record review and interview, caregiver did not obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.
Evidence:
1. Caregiver #2 's(Date of hire-07/02/18) record did not contain documentation of a minimum of sixteen clock hours of training. The record contained documentation of three hours of training.
2. Caregiver #1 confirmed that there was not a total of 16 hours of annual training for Caregiver #2.

Plan of Correction: The caregiver will complete the remaining hours of training.

Standard #: 8VAC20-800-240-A
Description: Based on observation and record review, the provider did not ensure all equipment inside of the home was maintained in a safe condition.
Evidence:
1. In the area where the children sleep, one of three cribs did not meet the U.S. Consumer Product Safety Commission's crib standards and is out of compliance. The manufacture date on the crib was April 28, 2010.
2. There was no further documentation to state that the crib was in compliance.

Plan of Correction: The crib will be removed from the home and a new one will be purchased to be in compliance.
