An unannounced monitoring inspection was conducted today from 10:00am-10:50am with the provider on site. There were 11 children with 4 caregivers. A sample of 2 children's files and 3 caregivers/household member's files were reviewed. The children were engaged in the following: playing outside with trucks and hula hoops. Interactions between the children and caregivers were positive. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. The physical space, attendance records, menu, drill record and the Emergency Preparedness Plan were reviewed. There were no medications on site. Standard #: 8VAC20-800-170-A Description: Based on review, the provider did not obtain from each caregiver at the time of hire prior to coming into contact with children a current Report of Tuberculosis Screening form or a form consistent with it documenting the absence of tuberculosis in a communicable form.
Evidence:
1. Caregiver #3 (start date 7/25/2022) did not have a Tuberculosis Screening form on file with the provider.Plan of Correction: The provider #3 will not be working on the daycare.
