Comments:

An unannounced renewal inspection was conducted on August 9, 2022 between 10:20am and 12:51pm. There were 3 children present with 1 caregiver supervising for a total of 8 points within the supervision guidelines. The children were observed playing beside each other, playing with blocks, an activity table, trucks, baby dolls and other age-appropriate toys. Positive interactions were observed during this inspection. The inspector reviewed compliance in the areas administration, physical plant, staffing and supervision, programing, medication, fire drill log, menu, attendance and emergency procedures. Children?s records, staff and household member records were reviewed. . Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the programs. Standard #:	8VAC20-800-180-A
Description:	Based on observation, the provider did not obtain a current Report of Tuberculosis Screening form as required every two years from the date of the first screening. Evidence: Caregiver #1 Tuberculosis Screening form (dated 8/6/2020) expired on 8/6/2022.
Plan of Correction:	I will go today to get a new TB shot.
Standard #:	8VAC20-800-320-B
Description:	Based on observation, child-resistant protective covers were not installed on all unused electrical outlets accessible to children under five years of age. Evidence: 2 outlets in bathroom used by children did not have protective covers and 6 outlets in the crib / sleeping room did not have protective covers.
Plan of Correction:	I have ordered different ones that I think will be better.
Standard #:	8VAC20-800-690-D
Description:	Based on observation and interview with the provider, the diaper changing pad was not cleaned and sanitized after each use. Evidence: 1. The provider acknowledged she does not use bleach water solution to sanitize the diapering pad. She only uses a cleaning wipe. 2.The provider?s method does not meet the definition of cleaned and sanitized as stated in 8VAC20-800-10 3. A cleaning wipe was used to clean the diapering pad.
Plan of Correction:	I will start using again. I will let parent know.
