Discussed with provider: inspection posting requirement, menu, MAT training, emergency drills, modified license

Comments:
An unannounced monitoring inspection was conducted on 09/09/2022 between the hours of approximately 2:55 p.m. and 4:25 p.m. There were 8 children present with the Provider and 2 assistants for a total of 22 points within the supervision guidelines. The children were observed listening to a story and engaging in free play. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, menu, attendance and emergency procedures were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 22.1-289.058
Description: Based on observation and caregiver interview, the family day home did not have a working carbon monoxide detector.

Evidence: Caregiver #1 stated that there was not a carbon monoxide detector in the home.

Plan of Correction: I will order one today.

Standard #: 8VAC20-800-770
Description: Based on observation and caregiver interview, a working portable battery-operated weather band radio was not available to caregivers at all times.

Evidence: The provider did not have a portable battery-operated weather band radio in the home.

Plan of Correction: I will order one today.

Standard #: 8VAC20-800-50-A
Description: Based on record review, the family day home did not keep a written record of children in attendance each day.

Evidence: The last day that the family day home recorded attendance was 09/07/2022.

Plan of Correction: I will do attendance every day.

Standard #: 8VAC20-800-180-A
Description: Based on record review, the provider did not obtain for each adult household member a current Report of Tuberculosis Screening form every two years from the date of the first screening.

Evidence:
1. The most recent Report of Tuberculosis Screening form for Household Member #1 was dated 11/11/2019.

Plan of Correction: They have an appointment scheduled for next Tuesday.

Standard #: 8VAC20-800-240-A
Description: Based on observation, areas and furnishings of the family day home, inside, were not maintained in a clean, safe and operable condition.

Evidence:
1. There was chipped paint on the wall by the baby gate between the main daycare room and the area with the high chairs.
2. There was chipped paint on the wall by the cubby shelves in the main daycare room.
3. There was chipped paint on the baseboards by the television in the main daycare room.
4. There was chipped paint on the column in the middle of the main daycare room.

Plan of Correction: I have a painter coming soon.

Standard #: 8VAC20-800-320-A
Description: Based on observation, not all small electrical appliances were unplugged while not in use by the caregiver or with children under close supervision.

Evidence: In the kitchen, a toaster, toaster oven and coffee pot were plugged in while not in use.

Plan of Correction: I will unplug them.

Standard #: 8VAC20-800-690-D
Description: Based on observation, the diapering surface was not nonabsorbent and washable and was not cleaned and sanitized after each use.

Evidence:
1. The diaper changing pad had a cloth cover.

Plan of Correction: We won't use it anymore.
