Comments:
A renewal inspection was initiated on 9/8/2022 and concluded on 9/8/2022. There were a total of 7 children ( 16 points) observed with 2caregivers present. The children were engaged in the following: toy play, games, learning about shapes, coloring, hand washing, and other organized activities. The inspector reviewed compliance in the areas of administration, personnel and household members, physical environment, care of children, handwashing, diapering, medication, emergencies and nutrition. A total of 3 child records and 4 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-800-750-A
Description: Based on file review, there was no annual written parental authorization for topical skin products to be used.

Evidence: There was no annual written parental authorization for topical skin products to be used for Child #1.

Plan of Correction: Provider will obtain written parental authorization for when topical skin products are used.

Standard #: 8VAC20-800-760-A-1
Description: Based on observation, the first aid kit did not contain more than 1 triangular bandages.

Evidence: The first aid kit only contained 1 triangular bandage.

Plan of Correction: Provider will purchase another triangular bandage for the first aid kit.

Standard #: 8VAC20-800-830-B
Description: Based on review of the emergency response drill log, shelter-in-place procedures were not practiced a minimum of twice per year.

Evidence: The emergency response drill log did not contain documentation of shelter-in-place drills.

Plan of Correction: Provider will practice shelter-in-place drills twice per year going forward.

Description: Based on file review, a child's emergency contact information did not contain names of people other than the custodial parents who are authorized to pick up the child.

Evidence: Emergency contact information for Child #2 and Child #3 did not contain names of people other than the custodial parents who are authorized to pick up the child.

Plan of Correction: Provider will obtain names of persons other than the custodial parents who are authorized to pick up the child.

Standard #: 8VAC20-800-210-A
Description: Based on file review, caregivers did not obtain a minimum of 16 clock hours of training annually in areas relevant to their job responsibilities.

Evidence: There was no documentation of annual training for either caregiver for 2021.

Plan of Correction: Provider will ensure that all caregivers will obtain a minimum of 16 clock hours of training for 2022 and going forward.

Standard #: 8VAC20-800-320-B
Description: Based on observation, child-resistant covers were not installed on all unused electrical outlets and surge protectors accessible to children under 5 years of age.

Evidence: Electrical outlet near rolled up mats did not have a child-resistant cover installed.

Plan of Correction: Provider covered the socket with a protective cover during the inspection.

Standard #: 8VAC20-800-430-C
Description: Based on observation, children over the age of 2 had access to stairs with 3 or more risers because the protective barrier was not in operable condition.

Evidence: The gate at the bottom of the basement stairs was being held closed with a bungie cord instead of the original latch. The gate could be pulled open with little force and could snap the gate shut on a child.

Plan of Correction: Provider will have gate installed so that it can be properly latched closed.

Standard #: 8VAC20-800-690-D
Description: Based on observation, the diapering area was not nonabsorbent and washable.

Evidence: The diapering pad was torn, exposing absorbent foam.

Plan of Correction: Provider will purchase a replacement diapering pad.
