A renewal inspection was conducted today. The provider and two assistant caregivers were caring for 11 children, totaling 37 points. This inspection took place in the middle of the day. The children were eating lunch and preparing for nap. The children were served fish sticks, cheese sandwiches, pears, bananas, and milk for lunch today.
The Inspector reviewed compliance in the areas of administrative records, caregiver training, the physical plant, nutrition, background checks, health records, children's records, emergency preparedness, and adult records.

Standard #: 8VAC20-800-110-B Description: Based on a review of children's records, it was determined one immunization record was not completed by the physician as required.
Evidence:
Child record B does not include the physician's signature on the immunization record.Plan of Correction: I will remind the parent to have the shot record signed.
