An unannounced monitoring inspection was conducted with the provider on September 12, 2022. There were 11 children, ages, 4 months, 5 months, 13 months, 23 months, 2, 2, 2, 2, 3, 4, and 4 years old in the care of the provider and two assistants for a total of 29 points. The children were observed eating breakfast and playing with toys in the upstairs living room area. Five children's records were reviewed. Two staff records were reviewed and in compliance. There were no medications on site to be reviewed. The first aid kit, flashlight, and battery operated radio were observed and in compliance. The most recent emergency escape drill was documented as 7/20/22. The most recent shelter-in-place drill was documented as 10/8/2021.

Standard #: 8VAC20-800-100-A Description: Based on a review of five children's records, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: Child A (start date 8/10/22) did not have documentation of a physical examination in their record. Plan of Correction: The provider stated that she would request the missing documentation from the parent and place it in the child's record.
