Comments:
An unannounced monitoring inspection was conducted on 10/12/22. The provider and another caregiver was present with 13 children , a total of 36 points. The children were observed eating lunch and getting ready for nap time as well as nap time. The children had Mac and cheese, salad and fruit for lunch. Interaction between caregivers and children observed during the inspection. A violation notice was given to the provider along with this inspection summary. Standard #: 8VAC20-800-100-A
Description: Based on the review of children's records the provider failed to get information that is needed.
Evidence: Reviewed 4 children's records, Child 1 is missing a physical.

Plan of Correction: I will ask the parent for the physical

Standard #: 8VAC20-800-30-A
Description: Based on the observation during the inspection, the provider failed to comply with the terms of her license and local laws.
Evidence: At the time of the inspection the provider had 13 children in care.

Plan of Correction: I will make sure I do not have more than the allowed children in care. A part time child care today because we were closed Monday and another child also showed up. I thought the full time child would not be here today.

Standard #: 8VAC20-800-40-A
Description: Based on the observation during the inspection, the provider failed to stay within the maximum capacity of the license.
Evidence: The provider had 13 children in care and is licensed for 12.

Plan of Correction: I will make sure I stay within my licensed capacity.

Standard #: 8VAC20-800-140-B-3
Description: Based on the review of caregiver records the provider failed to have certification of CPR training available for review.
Evidence:Based on the review of the providers training, the provider does not have a current CPR certificate available for view.
The two assistants both have CPR certification that has expired.
Caregiver 2 expired 10/7/22
Caregiver 3 expired 8/25/22

Plan of Correction: I think we took this a few month ago and I will send you the certificates, if not we will take it.

Standard #: 8VAC20-800-140-B-4
Description: Based on the review of caregiver records, the provider could not produce the paperwork for staff that are currently certified in first aid.
Based on the review of the providers training, the provider could not show current certification in first aid.
The first aid certificates for caregiver 2 and 3 are expired. Staff 2 expired 10/7/22 and staff 3 expired 8/25/2022

Plan of Correction: I think we took this a few month ago and I will send you the certificates, if not we will take it.

Standard #: 8VAC20-800-570-A
Description: Based on the observation, the provider failed to have an additional caregiver and was over the 32 points allowed with two caregivers.
Evidence:
The provider had 13 children in care for a total of 36 points. She was over by 4 points at the time of the inspection.

Plan of Correction: My husband had to go out of town for an emergency, he is usually here as a caregiver. I will make sure I stay within the 32 points when I have one assistant.
