Technical Assistance:
Discussion of daily attendance in regards of checking names off versus listing the name of each child with the date and time-in and time-out.

The Provider has also submitted the approval documentation from the Town of Leesburg to increase the posted capacity of the FDH to the new total capacity of (no more than) 12 children (at any one time). This home occupation permit was signed off on 10/17/2022.

Comments:
This renewal inspection was initiated by licensing staff and concluded on 10/14/2022, between the hours of 10 am to 12 pm. There were 2 preschool-age children present (4 points) with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 2 children records, 1 caregiver record, and 3 staff records were reviewed.

Information gathered during the inspection determined that there were (3) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home.

Standard #: 8VAC20-800-60-B
Description: Based on review, 2 of 2 children files indicated that not all of the required information was contained within each child's record.

Evidence -
1. On the date of inspection (10/14/2022), the action for the allergy listed was not available for review for the following children: Child #1, and Child #2.
2. The need of the action for allergy listed was previously cited for Child #1 during a monitoring inspection conducted on 09/28/2022.

Plan of Correction: All missing documentation and/or information will be completed.

Standard #: 8VAC20-800-80-A
Description: Based on review, 1 of 2 children files indicated that the verification of each child's proof of identification was not on file.

Evidence -
1. On the date of inspection (10/14/2022), the proof of identification was not available for Child #2.
2. The start date listed for Child #2 was listed as 10/01/2022. Child #2 was observed on site on the date of inspection.

Plan of Correction: All missing documentation and/or information will be completed.

Standard #: 8VAC20-800-90-A
Description: Based on review, 1 of 2 children files indicated that the Provider did not obtain documentation that a child had been adequately immunized (according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations) before attending the Family Day Home (FDH).

Evidence -
1. On the date of inspection (10/14/2022), and immunization page was not available for Child #2.
2. The start date for Child #2 was listed as - 10/01/2022.
