Comments:
An unannounced monitoring inspection was conducted on 10/19/2022 from 9:40am-11:30am. There were 9 children (22 points) supervised by 2 direct caregivers. The home, 3 staff records, 5 children?s records, 3 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed engaging freely and singing. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Violations:
Standard #: 8VAC20-800-760-A-1
Description: Based on observation, the center did not ensure each first aid kit had all required items.

Evidence: Adhesive tape and tweezers were missing from the first aid kit.

Plan of Correction: indicated items are already added in first aid kit.

Standard #: 8VAC20-800-770
Description: Based on observation and interview, the family day home did not have a working portable battery-operated weather band radio and extra batteries.

Evidence:
1. During the inspection, a portable battery-operated weather band radio was available, but it was inoperable.
2. The caregivers stated that no extra batteries were available.

Plan of Correction: Radio in operable now. Batteries are replaced.

Standard #: 8VAC20-800-780-B
Description: Based on record review, the Provider did not review with the parent the emergency contact information to ensure the information is correct and did not obtain the parent?s signed acknowledgment of the review, annually.

Evidence:
1. Child #3 emergency contact information was last reviewed on 1/26/2021.
2. Child #4 emergency contact information was last reviewed on 3/15/2021.

Plan of Correction: Emergency contact info is updated on child #4 and #3.

Standard #: 8VAC20-800-120-A
Description: Based on record review and interview, A caregiver?s record was missing two references.

Evidence: Caregiver #2 record is missing documentation that two or more references as to character, reputation, and competency were checked before employment. The employment date of caregiver #2 is 9/22/2022.

Plan of Correction: She is not working anymore since 10/31/202

Standard #: 8VAC20-800-70-A
Description: Based on record review, the Provider did not ensure that parents were provided in
writing whether or not there is liability insurance of at least $100,000 per occurrence and $300,000 aggregate in force on the family day home operation as required by ? 22.1-289.050 of the Code of Virginia

Evidence: There was no documentation in Child #5's record indicating that the family day home informed the parent or guardian of the family day home's liability insurance status.

Plan of Correction: Indicated item is already included.

Standard #: 8VAC20-800-140-A
Description: Based on record review and interview, The Substitute Provider failed to record and sign their arrival and departure times for each day worked.

Evidence:
1.There was no documentation of arrival and departure times for each day worked for caregiver #3.

2. Caregiver #3 stated that they did not document their arrival and departure times for each day worked.

Plan of Correction: Indicated item is already corrected she is not working anymore.

Standard #: 8VAC20-800-170-A
Description: Based on record review and interview, The provider did not obtain from each caregiver at the time of hire a current Report of Tuberculosis Screening form published by the Virginia Department of Health.

Evidence: There was no documentation of a tuberculosis screening for caregiver #2. The employment date of caregiver #2 is 9/22/2022.

Plan of Correction: Not applicable anymore she is not working anymore.

Standard #: 8VAC20-800-200-A
Description: Based on record review and interview, the Provider did not ensure all caregivers had obtained by the end of their first week of assuming job responsibilities the required orientation.

Evidence: There was no documentation showing that caregiver #2 had received orientation training by the end of their first week. The employment date for caregiver #2 is 9/22/22.

Plan of Correction: Verbal orientation was provided and not working anymore.

Standard #: 8VAC20-800-240-A
Description: Areas and furnishings of the family day home inside was not maintained in a clean, safe, and operable condition.

Evidence: In the playroom, there were several areas on the wall near the bathroom and garage door where the paint was chipping.

Plan of Correction: It is clean now and will fix painting

Standard #: 8VAC20-800-330-A
Description: Based on observation and interview, a landline telephone was not operable during the family day home?s hours of operation.

Evidence: The provider had a landline phone, but it was inoperable.

Plan of Correction: It in operable now.
