Discussed the importance of keeping all small appliances unplugged when not in use.

Routinely monitoring the name labels on cots used for naps will ensure that the names remain current for use.

Comments:
An unannounced Renewal inspection was completed today. A total of 11 children (26pts) with the Provider and 1 Assistant were present today. The children were observed in two groups, one grouping designated for the younger children. They were observed sitting at a table with the Assistant, engaged in table top activities. The older children were observed on the floor playing in smaller groups and were laying patterns out with magnets and sorting colors in muffin tins. Positive interaction was observed by staff with the children. A nice transition to a gross motor activity that consisted of dancing and jumping to music was completed. Walk thru of the upper level of the home and the lower level used for programming was conducted. A total of 6 children's records, all household records and all staff records (2) were reviewed. No medication needs at this time and no known food allergies. This inspection began at 10:15am and ended at 12:10pm. Thank you for your cooperation today. Standard #: 8VAC20-800-50-A
Description: Based on observation, a written record of children in attendance was not maintained.
Evidence:
On the day of the inspection a written record of attendance was not completed.

Plan of Correction: I completed the attendance for today during the inspection.

Standard #: 8VAC20-800-60-A
Description: Based on child record review, 4 out of 6 children's records were incomplete.
Evidence:
1. The medication policy for Child #2 was not completed.
2. Child #3's record did not contain the provider's provisions of the emergency preparedness plan.
3. The Information to Parents was not signed in Child #6's record.
4. Child #5 began care on 09/06/2022. The authorization for emergency medical care was not obtained until 09/12/2022

Plan of Correction: I will complete and have signatures for all these documents.

Standard #: 8VAC20-800-80-A
Description: Based on child record review, the provider did not obtain proof of identity for one child within 7 days of the child's first day of attendance.
Evidence:
The provider did not obtain and review the proof of identity for Child #2. who began care on 10/03/2022

Plan of Correction: I will review the child's birth certificate and document the review and date it.

Standard #: 8VAC20-800-90-A
Description: Based on child record review, the provider did not obtain documentation that 2 children have been adequately immunized before attending the family day home.
Evidence:
Child #2 began care on 10/03/2022, documentation of immunizations were not obtained until 10/17/2022. Child #4 began care on 09/07/2022, documentation that Child #4 was adequately immunized was not obtained.

Plan of Correction: I will obtain a current record of immunizations for Child #4. Immunizations will be obtained before a child will begin care.

Standard #: 8VAC20-800-280
Description: Based on observation, cleaning agents and other poisonous substances were accessible to children.
Evidence:
The closet located in the lower level bathroom was unlocked and had cleaning products and cans of paint that were within reach of children in care.

Plan of Correction: I locked the closet door immediately.
