Comments:
An unannounced monitoring inspection was conducted on October 27, 2022. There were 6 children present (19 points) with two caregivers supervising. The children were observed singing songs, working on colors in English and Spanish and playing with floor toys. The home, children's records, caregiver records, evacuation logs, attendance logs, first aid supplies, menu's, cots and napping areas were reviewed. The children had a breakfast that consisted of Oatmeal, pancakes, fruit and milk. Areas of non-compliance were cited on the violation notice. Standard #: 8VAC20-800-60-B
Description: Based on a review of children's records, the provider failed to obtain required documentation for each child enrolled.

Evidence:
Child #2's record does not contain documentation of proof of the child's age and identity.
Child #3's record does not contain documentation o of the review of the child's emergency contact information annually.
Child #4's record does not contain documentation of phone numbers of two designated person to contact in case of an emergency or the names of persons other the custodial parents who are authorized to pick up the child.
Child #5's record does not contain documentation of the telephone number of each parent's place of employment, the name office address and telephone number of the child's physician or documentation of the first date of attendance.

Plan of Correction: I will get the information from the families and put it in the children's files.

Standard #: 8VAC20-800-210-A
Description: Based on a review of caregiver records, the provider failed to ensue that all caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Evidence:
Caregiver #2's record did not have documentation of obtaining 16 clock hours of relevant training annually.

Plan of Correction: The caregiver will complete all training hours by December 2022.
