Exemption form for immunizations will be forwarded to the provider as a resource. The Information to Parents form will be forwarded to the provider. You are reminded that a monthly fire inspection is required and 2 shelter in place drills annually is also required.

Description:	Based on staff statement and review of the evacuation drill log, monthly evacuation drills were not completed. Evidence: The provider stated a September and October evacuation drill was not completed or documented.
Plan of Correction:	A fire drill will be completed in November and monthly from now on.
Standard #:	8VAC20-800-70-A
Description:	Based on review of child records, information to parents was not provided before attendance for 5 children. Evidence: The following information was not provided to the parents of Child #1, 3, 4, 5 and 6: 1. Policies for termination 2. Amount of time a substitute provider may be onsite 3. Notification of injuries, illness, behavior problems, when first aid is administered, lost or death of a child, communicable diseases and daily communication.
Plan of Correction:	I will have each parent review the second page of the Information to Parents form, sign and date.
Standard #:	8VAC20-800-90-A
Description:	Based on review of child records, documentation that a child has been adequately immunized was not obtained prior to beginning care. Evidence: Child #3 began care on 10/03/2022. Documentation that the child has been adequately immunized was not obtained.
Plan of Correction:	This child is religiously exempt from immunizations. I will have the parent sign the exemption form.
