Comments:

An Unannounced Renewal Inspection was conducted on 11/9/2022. Upon arrival at 10:36 a.m. there were a total of 8 children, ages 5 months to 3 years, with a total of 23 points under the care of two staff . The Licensing Inspector reviewed four children?s records and three household/staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:

Standard #:	8VAC20-800-210-A
Description:	Based on record review, the Licensee failed to ensure that each caregiver has completed a minimum of 16 hours of annual training. EVIDENCE: Record for Staff #3, did not include documentation to support that required training has been completed within the past year. The most recent training document in the file is dated 7/14/2020.
Plan of Correction:	Staff will complete required 16 hours annual training for year 2021. An additional 16 hours of annual training will be completed for year 2022.
Standard #:	8VAC20-800-240-A
Description:	Based on observation and interview, the Licensee failed to maintain areas of the family day home in a safe condition. EVIDENCE: 1. A gate at the entryway of the home to keep children away from an outside door had vertical slats 3 ? inches wide, which is wide enough for a small child?s head to become entrapped. 2. A home repair involving removing part of the ceiling was completed about a month ago, according to Staff #1, and the ceiling is still uncovered with insulation hanging down. 3. A panel covering the HVAC system is not secured to the wall and easily tips over into the hall opposite the bathroom that is used by children.
Plan of Correction:	Provider will add make necessary changes.
Standard #:	8VAC20-800-250-A
Description:	Based on observation, the Licensee failed to have window blind cords kept out of reach of children under five years of age. EVIDENCE: Loose cords used to operate a window blind were hanging down within reach of children. During the inspection, all children were under the age of five years.
Plan of Correction:	Provider will readjust curtain cords to make sure children do not have access to them. Corrected during inspection.
