Technical Assistance:
Discussed the new form for Sworn Disclosure statements. Discussed the Supplemental page should not be posted. Discussed the process to follow up on Central Registry results if not received within 30 days.

Comments:
An unannounced monitoring visit was conducted today between the hours of 9:15am through 10:00am with the provider on site. There were 10 children with the provider and two assistants. A sample of 2 children's files and 3 staff/household member's files were reviewed. The children were engaged in the following: toy play, reading books, bathroom breaks, hand washing, and sleeping. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. Interactions between the children and staff were positive. The physical space, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. There were no violations cited this date.
