Technical Assistance:
Technical assistance was offered related to: Cribs, food from home, emergency drills, staff trainings.

Comments:
A monitoring inspection was conducted on 3/15/23 from 10:55 am to 12:45pm. There were 8 children (23 points) present, with 3 caregivers supervising. The children were observed playing with toys, reviewing numbers and colors, and eating lunch. Handwashing was observed prior to lunch. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, special care and emergencies and nutrition. A total of 2 children's records, 1caregiver's record, attendance and emergency drill records were reviewed. There were no medications on site.

Violations:
Standard #: 8VAC20-800-830-A
Description: Based on staff interview, emergency evacuation procedures were not practiced monthly with all caregivers and children in care.
Evidence: The provider stated that evacuation drills were not conducted for January and February of 2023.

Plan of Correction: I will make sure that evacuation drills are conducted each month.

Standard #: 8VAC20-800-830-C
Description: Based on staff interview and record review, documentation was not maintained of emergency evacuation drills.
Evidence:
1. The evacuation drill log had documentation only for December 2022.
2. The provider stated that she had conducted the drills but forgot to document them in the log.

Plan of Correction: Documentation for all emergency drills will be completed and accurately documented on the log.

Standard #: 8VAC20-800-960-E
Description: Based on observation, bottles were not labeled with the child's full name and the date with more than one infant in care.
Evidence:
1. During the inspection, two infants were present.
2. A used milk bottle for Child # 1 was not labeled with his name and date.

Plan of Correction: Bottles for all infants will be labeled fully.

Standard #: 8VAC20-800-120-B
Description: Based on staff interview and records review, the record for an assistant was not complete.
Evidence:
1. Caregiver # 1's first day of employment was 1/11/23. Documentation of two reference checks were not obtained to this date for Caregiver # 1.
2. The provider stated that she had given Caregiver # 1 the reference check forms but did not receive them back.

Plan of Correction: References will be obtained.

Standard #: 8VAC20-800-240-A
Description: Based on observation, areas inside the family day home were not maintained in a safe condition.
Evidence: In the kitchen on the main level, three lit candles were found on a tray. The candles were unattended to, posing a fire and safety hazard.

Plan of Correction: The candles were extinguished during the inspection. Candles will not be lit and left unattended while children are in care.

Standard #: 8VAC20-800-320-A
Description: Based on observation, small electrical appliances were not unplugged when not in use.
Evidence: An electric kettle and a coffee machine was left plugged in the kitchen on the main level of the home. The appliances were not in use.

Plan of Correction: Electrical appliances will be checked to make sure they are unplugged as soon as they are not in use.
