Comments:
An unannounced renewal inspection was conducted today between 10:50am and 12:30pm. There were 11 children present with the Provider and 2 assistant for a total of 26 points within the supervision guidelines. The children were observed working on art, playing with puppets, building with blocks, eating lunch (chicken noodle soup, strawberries and milk) and other organized activities. Handwashing and diaper changing procedures were also observed. There is an ample amount of toys and supplies available to the children. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Violations:
Standard #: 8VAC20-800-960-E
Description: Based on observation bottles were not labeled with the date.
Evidence: Child #1 had a bottle observed in the family day home refrigerator. The bottle was not dated.

Plan of Correction: We must have forgotten. We will label the bottles.

Standard #: 8VAC20-800-70-A
Description: Based on record review, parents were not provided in writing whether or not there is liability insurance on the family day home operation as required by ? 22.1-289.050 of the Code of Virginia before each child's first day of attendance.
Evidence: Child #1 and Child #3's records do not contain documentation that the parents have been informed of the family day homes insurance declaration.

Plan of Correction: I have the forms. I can have the parents sign it today at pickup.

Standard #: 8VAC20-800-280
Description: Based on observation, potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents and disinfectants were not stored in areas inaccessible to children.
Evidence: Bleaching and other various cleaning agents were observed in the cabinet under the sink in the nap room bathroom. There were no lock on the cabinet or bathroom door.

Plan of Correction: The cleaning materials were moved during the inspection.

Standard #: 8VAC20-800-320-B
Description: Based on observation, child-resistant protective covers were not installed on all unused electrical outlets.
Evidence: Two outlets in the nap room and one outlet in the nap room bathroom were observed without outlet covers.

Plan of Correction: Corrected during the inspection.
