22VAC40-185 PHYSICAL PLANT.
The Licensing Inspector conducted a complaint investigation in response to a complaint that was received on 10-17-14. Staff interviews and physical plant observation was conducted relating to allegations of blocked toilets, broken soap and paper towel dispensers. The information gathered during this investigation supports the allegations, so the complaint is determined to be valid. The violation was discussed with the Center Director. Standard #: 22VAC40-185-270-A Complaint related: No Description: Based on observation and staff interviews, equipment of the center is not maintained in a operable condition. Evidence: Room #11 was inspected, the toilet located in the second stall was found clogged with toilet paper and not flushable. Staff stated that it has been broken for about a month. The soap dispenser located to the right of the sink was broken, when you pushed the handle the casing fell off, not allowing any soap to be dispensed. The paper towel dispensers above the classroom sinks in rooms #6 and #7 were broken, not allowing paper towels to be dispensed. Staff stated that the dispensers have been broken for about a month. Plan of Correction: Toilet was un-clogged. Soap dispenser fixed and paper towel dispenser fixed and replaced.
