Comments:
On January 27, 2023, the department received a complaint regarding injury to a child in the family day home. On January 30, 2023, the licensing inspector initiated a complaint inspection at approximately 11:00 a.m. along with representatives from two local agencies at the provider?s home. The provider was interviewed and observations were made in the home. Additional interviews, documentation review, and observations were conducted off-site. The inspection was completed 04/12/2023.
The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details.

Description: Based on interview and injury report, the provider failed to ensure that prescription and nonprescription medications shall be given to a child only with written parent permission.
Evidence:
The injury report for C1 dated 01/27/2023 documented ?scratches around eye and mouth? to which the provider applied ?Aquaphor (Neosporin)?. The child record did not contain written permission to apply this medication.

Description: Based on interviews and written documentation, the provider failed to ensure the home activities, services and facilities are conducive to the welfare of children in care.
Evidence:
The licensing office received notification of a child injury that occurred in the family day home 01/27/2023. During the inspection on 01/30/2023, the provider stated that C1 (age 7 months) fell asleep, that she placed C1 in a crib and then went to use the bathroom. The provider reported that while she was in the bathroom, she heard C1 crying. The provider exited the bathroom and observed scratches on C1?s face around the eyes and mouth that she had not previously observed.
