An investigation was conducted today from 9:45am-11:00am regarding an allegation of staff to child ratios for the Preschool and Pre-K classrooms. Interviews were conducted and documentation reviewed. It was deemed that a preponderance of evidence supports the allegation. Other non-complaint related violations were cited. Areas of non-compliance are identified in the Violation Notice. Description: Based on direct observation, hazardous substances were not kept in a locked place. Evidence: A cabinet containing Lysol spray and a bleach/water solution was observed unlocked in the Pre-K classroom. The cabinet was observed with a key in the locking device, but the door was open and not securely fastened in locked manner.

Plan of Correction: Cabinet was secured during the inspection.

Description: Based on direct observation, electrical outlets did not have protective covers. Evidence: Two outlets were observed without protective covers on the wall above the water fountain in the Pre-K classroom.

Description: Based on documentation review, the staff to child ratio of one staff member for every 10 children was not followed for children from three years to five years of age. Evidence: Upon review of the center's child tracking sheets, the staff to child ratios were not in compliance on the following days: 1. On November 5, 2014 at approximately 8:55am, one teacher was supervising 11 children for a duration of five minutes in the Pre-K classroom. 2. On the dates of November 7th, November 10th, November 11th, and November 12th, one teacher was supervising 12 children, at times for a duration up to one hour, in the Pre-K classroom. 3. On November 13, 2014 at approximately 9:41am, one teacher was supervising 11 for a duration of 40 minutes in the Preschool (3yrs) classroom. 4. On November 14, 2014 at approximately 9:35am, one teacher was supervising 12 children for a duration of 50 minutes in the Preschool (3yrs) classroom.

Plan of Correction: Immediately adjusted staff schedules to meet ratio requirements. Retrained staff on ratio, and CSR policies.
