An unannounced monitoring inspection was conducted on February 10, 2015 from approximately 10:30 am to 1:15 pm. Four staff were present with sixteen children. Additional staff were on site for administrative and kitchen duties. Children were observed during snack, circle time, free play and lunch. Two children?s records and three staff records were reviewed on this date. Licensing Inspector reviewed supervision, activities, equipment, diapering procedures, hand washing procedures, nutrition, special feedings, injury report documentation, children and staff records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, liability insurance coverage, emergency supplies and required postings.
Standard #: 22VAC40-185-210-A-4-a Description: Based on review, facility did not ensure staff designated as lead teacher have at least 24 hours of training when qualified under programmatic experience and training hours. Evidence: On February 10, 2015, the record for staff #3 stated the individual was promoted to lead teacher in December 2014. Staff completed 15.5 hours of training between June 2014 until current date. Plan of Correction: Staff will complete additional training hours.
Standard #: 22VAC40-185-260-A Description: Based on review, facility did not ensure there is an annual fire inspection conducted. Evidence: On February 10, 2015, the most recent fire inspection report available for review was conducted on December 4, 2013. Plan of Correction: Program Director has called and requested inspection already.
Standard #: 22VAC40-185-440-L Description: Based on observation, facility did not ensure filled comforters are not used by children under the age of two years. Evidence: On February 10, 2015, Licensing Inspector observed an infant sleeping in a crib with a filled comforter as a cover. Plan of Correction: Comforter was removed from the classroom.
