Comments:
An unannounced renewal inspection was conducted jointly with the licensing administrator on September 20, 2023, from 9:45am-11:15 am. At the time of entrance, 2 children ( 7) points were in care with 1 caregiver present. The inspector reviewed 5 children?s records. The caregiver and household member records are current at this time. Children were observed watching TV, being fed a bottle and diaper changing. No medications were on site. Areas of non-compliance are identified on the violation notice. Repeat violations were found and are identified within this report.
A 6 1/2 week old infant was found in care this day, the terms of the license is for children 3 months through 12 years. A violation was cited for not following the terms of your license. Please email the licensing inspector immediately to request the age range for your license to be lowered to accommodate the 6 1/2 week infant.
The carbon monoxide detector was not working and the inspector requested that the batteries get changed or a new one purchased.
There was a discussion about maintaining the capacity of 5 children at all times, the provider was considering enrolling another child.

The plan of correction was discussed during the inspection.

Description: Based on observation and provider interview, the menu for the current week is not posted or given to parents.
Evidence: The provider stated she does not have a menu, it is not posted nor given to the parents.

Plan of Correction: I will make a menu to post

Standard #: 8VAC20-800-60-B
Description: Repeat Violation:
Based on a review of children records, the provider has not obtained the required information for each child in attendance.
Evidence: The following information is missing for child # 3, #4, #5, all date of first attendance 9/11/2023.
b.Name, address, and telephone number of each custodial parent's place of employment:;
c.Telephone number of the child's physician;
f..Policy number of the child's medical insurance;
d.Name, address and telephone number of two designated persons to
contact in case of an emergency if the parent cannot be reached;
5. Proof of the child's age and identity to include date proofed and person viewing;
8. Written authorization for emergency medical care to include child name, provider's name and parent signature.

Plan of Correction: The parent will complete all the missing information.

Standard #: 8VAC20-800-70-A
Description: Based on a review of children records, the provider has not provided in writing the following information to parents before the child's first day of attendance.
Evidence: The following information is missing for child # 3, #4, #5, all date of first attendance 9/11/2023.
2. Schedule of fees and payment plans;
3. Check in and check out procedures;
5. Whether or not there is liability insurance to include child's name and parent signature and date;
11. General daily schedule that is appropriate for the age of the enrolling child;
16. Provisions of the family day home's emergency preparedness and response
plan to include name of provider and relocation name;
18. Policies for termination of care;

Plan of Correction: I will fill in all the missing information and have the parents sign the forms.

Standard #: 8VAC20-800-90-A
Description: Based on a review of children records, the provider has not obtained documentation that the child has been adequately immunized prior to attending the family day home.
Evidence: Child #3 and #4, date of first attendance 9/11/2023 does not have documentation of immunization records.

Plan of Correction: I will tell the parent that I need the immunization record.

Standard #: 8VAC20-800-240-A
Description: Based on observation and provider interview, the furnishings are not maintained in a safe and operable condition.
Evidence: The crib used by a 6 1/2 week infant is not maintained I a safe and operable condition. The side railing is pulled away from the lower portion of the main frame of the crib, leaving a gap that the infant can possible get stuck. The provider stated she has removed the bottom screws to allow the side panel to come up and over the top of the crib.

Plan of Correction: I will place the screws back into the crib so that all sides are fastened to the frame of the crib.

Standard #: 8VAC20-800-530-B
Description: Based on observation and provider interview, the crib being used by an infant does not have a tight-fitting bottom sheet.
Evidence: The 6 1/2 week old infant was placed in a crib without a fitted sheet. The infant was placed on a blanket that was being used for a bottom sheet.

Plan of Correction: I will use a fitted sheet on the crib.

Standard #: 8VAC20-820-120-A-6
Description: Based on observation and child record review, the provider is not operating within the terms of her license regarding the age range of children for whom care may be provided.
Evidence: The provider was observed bottle feeding an infant upon our arrival. The infant record was reviewed, the date of birth is 8/4/2023. The infant is 6 1/2 weeks old. The provider's license states she may care for children 3 months through 12 years.

Plan of Correction: The provider will request to change her age range to accommodate the 6 1/2 week infant.
