This monitoring inspection was initiated by licensing staff and concluded on 11/01/2023, between the hours of 1 pm to 3 pm. There were 10 children present (27 points), ranging from infant to preschool-age, with 4 caregivers supervising. The children were engaged in the following: diapering, bathroom breaks, hand washing, nap time, and other organized activities. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 11 children records, 4 household records, and 4 staff records were reviewed.
Information gathered during the inspection determined that there were (3) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home.

Standard #: 8VAC20-770-40-D-5 Description: Based on review, a household member, 14 years of age and older did not have a search of the central registry (CPS check) available.
Evidence -
1. On the date of inspection (11/01/2023) Household Member #4 did not have a CPS check available for review.
2. The Provider had made an error and obtained a current Fingerprint Check (FPC) instead of a search of the central registry. The FPC was reviewed and is on file.Plan of Correction: A new request of the central registry for HM #4 will be sent. These findings once obtained will be shared with the Licensing Inspector (LI).
Standard #: 8VAC20-800-80-A Description: Based on review, 1 of 11 children files indicated that a review of documentation for each child's proof of identification was performed within 7 business days of each child's first day of attendance by the Provider.
Evidence -
1. On the date of inspection (11/01/2023), documentation of the review of the Proof of Identification for Child #8 was not available.
2. The start date for Child #8 was listed as 01/18/2023.Plan of Correction: The Parent of this child will be asked to bring in this documentation for review. This review will be documented and/or the documentation will be copied and placed on file.
Standard #: 8VAC20-800-140-B-3 Description: Based on review, 1 of 4 staff files indicated that there is not a current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care.
Evidence -
1. On the date of inspection (11/01/2023), the dual certification in both CPR & First Aid for Staff #1 expired on 09/07/2023.
2. No other documentation to show a current certification status was available.Plan of Correction: A recertification class will be scheduled for both CPR and First Aid.
