22VAC40-191 Background Checks (22VAC40-191)

An unannounced renewal inspection was conducted today, April 14, 2015 to determine the center's compliance with licensing standards. The inspector arrived at 1:45 pm and concluded the inspection at 4:15 pm. . The children were engaged in various activities upon the inspectors arrival. In the Infant room, infants were being fed, rocked, napping and playing on the floor with soft blocks and musical toys. The staff held the infants and spoke to them in a soothing manner. The toddlers were observed dancing to music and playing with the kitchen set. Preschool children had circle time. Staff lead the children in an interactive song/ play, "going on a bear hunt". Children sang songs and marched around the circle time area. Children had completed art work making flowers using the print of a fork. The pre-k program was observed playing in centers. Children read books, played with the doll house, house keeping, dolls and marble tracks. Children were also reviewing the upper case letter "U". Later school age children arrived via public school bus and center van. Children had snack before starting homework and center time. The playground area was inspected. A new sandbox has been constructed along with a new playhouse on the playground. The fence surrounding the the play area has been modified with a protective covering along the top to ensure there are no sharp metal points exposed. The classrooms are decorated with the children's artwork and are equipped with age and stage appropriate materials. The center had the following posted: license, daily schedule, emergency numbers, evacuation maps (see violation notice), menu and various parent information. The center's first aid kit and emergency supplies were inspected and found complete. This week the facility is hosting "The Week of the Young Child" with the theme of take five and.... (play outside, play a game, color a picture, read a book and take a photo). Parents are encouraged to take five minutes and complete one of the listed activities when they arrive to pick up their child. A photo booth will be on cite on Friday for parents and children to have a photo taken to end the week. Four employee records and five children's records were reviewed during today's inspection. Emergency medications and authorizations were reviewed. Two staff have current EMAT certification. Health Inspection: 08/20/2014 Fire Inspection: 10/08/2014 The last documented evacuation drill was held on 03/17/2015. The two required annual shelter-in-place drills for 2014 were completed on 03/11/2014 and 07/09/2014. One shelter-in-place drill for 2015 was documented on 03/12/2015. Today the following ratios were observed: Pre-K: ( 4 and 5 year old) 12 children with 2 staff. Toddlers: 12 toddlers with 3 staff. Preschool: (2 and 3 year old) 14 children with 3 staff. Infants: 7 infants with 2 staff. School age children: 36 children with 3 staff. The monthly menu was posted and today's lunch consisted of: grilled cheese , mixed vegetables, oranges and milk. The violations from the previous inspection on October 6, 2014 were checked for corrections. There was one repeat violations found during today's inspection. The Department has received a complete renewal application and a subsequent license will be recommended. Description: Based on observations, the facility did not ensure all area and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: One post of the fence that surrounds the preschool playground has the cap to the top of the post missing, exposing on screw with a sharp point. The point of the screw could snag the skin of a child in care.

Plan of Correction: Per the director: I will remove the screw from the fence.

Description: Based on observations, the facility did not post emergency evacuation and shelter-in-place procedures/maps in a location conspicuous to staff and children on each floor of each building. Evidence: Maps and procedures for emergency evacuation were posted however the they did not contain information regarding the procedures and designated area for shelter-in-place.

Plan of Correction: Per the director: I will add to the emergency procedures and maps the designated area to shelter-in-place and procedures to be taken.
