Standard #: 22VAC40-185-70-A
Description: Based on staff interviews, the center had 5 student volunteers helping in the center and there were no records maintained on these staff volunteers. Evidence: 1. Two staff were interviewed and both confirmed that there were 5 students from NOVA Community College that regularily helped in the center. 2. One staff stated these students did not have files and that references were not conducted. It was also acknowledged that these staff were immediately released by upper management once it was learned that these volunteers were present without paperwork.

Plan of Correction: One April 15, all volunteer activities were halted. All volunteers will have a file maintained that includes all necessary paperwork before they can assist in the classroom..

Standard #: 22VAC40-185-270-A
Description: Based on staff interviews, it was confirmed that the outside playground was not maintained in a clean and safe condition when a dead mouse was found on the playground by a child and picked up by the child. Evidence: 1. Center has a policy that staff observe the playground before children go out to check for any possible hazard. 2. Staff interviewed stated this incident took place about a week ago and and that they did not notice the dead mouse until they saw the child holding it by the tail.

Plan of Correction: We contacted the health department and contacted exterminators to come out. We will check the playground every day in the morning for possible hazards.

Standard #: 22VAC40-185-350-C
Description: Based on staff interviews, the center was out of ratio for approximately 45 minutes on April 14, 2015, when one staff was alone supervising 24 children, ages 2 years through 11 years. Evidence: 1. Two staff were interviewed and both staff confirmed that the center was short a staff member from 630 until 740. 2. The staff also confirmed that one staff was alone with a group of children ranging from 2 years to 12 years. The group grew from 4 to 24 during that time and only one staff was supervising them.

Plan of Correction: Administration has taken steps to correct this.
