An unannounced monitoring inspection was conducted today from 10:25am-12:30pm. There were 193 (infants-5yrs.) directly supervised by 27 staff. The physical plant, 1 staff record, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing outside, and eating snack. Diapering and hand-washing procedures were also observed. There was an abundant supply of books, toys, and materials for the children. The center was clean and organized. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Standard #: 22VAC40-185-60-A Description: Based on review, the facility failed to maintain children's records with documentation of proof of identity. Evidence: Reviewed 5 children's records and found that CH2 did not have documentation of the review/verification of the child's proof of identity. Plan of Correction: Parent of CH2 was contacted and asked to return proof of identity within 5 business days.
Standard #: 22VAC40-185-280-B Description: Based on observation, the facility failed to ensure that hazardous substances are kept in a locked place using a safe locking method that prevents access by children. Evidence: 1) Observed Spritz RTU Sanitizer in an unlocked cabinet in the main level children's bathroom. Observed disinfecting wipes in an unlocked cabinet in the main level children's bathroom. 2) Observed Oxivir Tb sanitizer in an unlocked cabinet in Room 6. 3) Observed Floor buff and mop spray accessible to children in the rear "closet" of Room 5. 4) Observed rubbing alcohol, Oxivir Tb and WD40 in unlocked cabinets in the Art Room. Plan of Correction: Memo sent to staff reminding them to always keep hazardous substances in a locked cabinet.
Standard #: 22VAC40-185-430-B Description: Based on observation, the facility failed to ensure that materials and equipment available are age and stage appropriate. Evidence: 1) Observed in Room 9, children between the ages of 2-3yrs rubber bands on the shelf accessible to the children in the room and small foam pieces causing a potential choking hazard. 2) Observed in Room 10 (toddlers) small Easter ornaments with string loops, small plastic flowers, and small Easter eggs that were small enough to be swallowed and cause a potential choking hazard for the children in the room. Plan of Correction: Rubber bands and small items were removed from both classrooms.
Standard #: 22VAC40-185-550-M Description: Based on review, the facility failed to ensure that children's injury records were maintained with the required information. Evidence: Reviewed 5 children's injury records and found that 1 did not have the time that parents were notified of their child's injury, and 2 were missing documentation of future action to prevent recurrence. Plan of Correction: Memo sent to staff reminding them to fully complete injury report form.
