Evidence: The LI reviewed prescribed medications for 3 children. The LI noted a discrepancy in the pharmacy label and the written parental and physician's authorization in regards to the prescriber's frequency of administration for one of the three. The pharmacy label on the medication for child a. noted 2 puffs every 6 hours as needed, the written authorization noted two puffs every 4 hours as needed. Plan of Correction: The Health Coordinator will review the medication and written authorization form with the child's parent to ensure the frequency of administration is the same as the pharmacy label or the child's physician will be requested to up date and authorize a change in the times the medication is to be administered.
