Comments:
An unannounced Subsidy Health and Safety inspection was conducted today at approximately 1:45pm . There was 4 children present directly supervised by the subsidy Vendor. Children were observed playing freely and watching children's programming. The home, emergency supplies and plans were inspected. Children?s information, vendor records and household member documentation was also reviewed. Per Vendor, there were no medications on site. Standard #: VENDHOM-000-005-B
Description: Based on review of attendance it was determined that a written hard copy record of daily attendance was not properly maintained. Evidence: During inspection the last date documented for attendance was 2 months prior to inspection

Plan of Correction: Ok, I will keep it up

Standard #: VENDHOM-000-006-B
Description: Based on review of child documentation it was determined that all required items were not maintained. Evidence: During inspection Child1 was missing: address for emergency contact, child care agreements and permission for field trips. Child 2 was missing: Home address, parents work place and phone numbers emergency contacts, authorized pick up person(s), child care agreements and permission for field trips. Child3 was missing: child care agreements and permission for field trips. Child4 and Child5 were missing:parents work place and phone numbers emergency contacts, authorized pick up person(s), child care agreements and permission for field trips.

Plan of Correction: I will talk to the parents

Description: Based on observation it was determined that Vendor did not wash hands as required. Evidence: During inspection vendor did not wash hands prior to providing snack to 2 of 4 children in care

Plan of Correction: Ok, I will hand were washed before you ( inspector ) came

Standard #: VENDHOM-000-029-C
Description: Based on review of Family Day Home it was determined that required information was not posted in the home as required. Evidence: During inspection emergency evacuation and shelter-in-place procedures/maps were not posted in the home

Plan of Correction: Vendor had it stored in a box and posted it during inspection once mentioned by inspector.

Standard #: VENDHOM-000-030-A
Description: Based on request and observation, it was determined that emergency drills were not conducted as required. Evidence: During inspection vendor did not have evacuation drills documented for the current calendar year.

Plan of Correction: I will update it.
